Vision Insurance
 

The District offers vision insurance through Avesis.  All regular employees working at least 20 hours or more per week are eligible to purchase this benefit.  The benefits are:

  • Eye exam once every year                              

  • Lenses once every year

  • Frames once every two (2) years

  • May choose eye glasses or contacts (not both in a plan year- April 1 through March 31)

 

  • Eyeglasses                              

    • $0 co-pay for eye exam                    

    • $0 co-pay for materials

    • If you stay within the plan limits, nothing other than eye exam & material co-pays is owed on glasses.

    • If you choose frames/lenses that exceed the limits allowed ($55 wholesale), you will pay the difference

  • Contact Lenses:                                      

    • $0 co-pay for eye exam

    • Receive a flat $130 per plan year to use towards the materials (i.e. contact lenses) and services (i.e. fittings)

  • If you choose to see a non-network provider, you would have to pay first, then file your claim.  This option will cost you more out-of-pocket.

To access your benefits:

  1. Contact a participating provider and make your appointment.

  2. Present your Avesis ID card to your provider

This is a voluntary benefit and the premiums are paid by the employee, if coverage is elected.  The premiums listed above are per paycheck, twice a month - 15th and the last day.
