The District offers Short Term and Long Term Disability insurance through UNUM Provident.  Disability helps to supplement your income when you unable to work due to a non-work related illness or injury.  All regular employees working at least 20 hours or more per week are eligible to purchase this benefit.

Short Term Disability

  • Benefit - 60% of salary; paid weekly

  • Maternity treated as any other illness

  • Choice of Options:

    • 14-day Elimination Period; 24 week benefit

    • 60-day Elimination Period; 18 weeks benefit

  • Benefit is offset by any sick leave benefits

  • Guarantee of coverage, if enrolled within 30 days of employment, otherwise, only during Open Enrollment and medical underwriting is required.  As a result, coverage can be denied.

Long Term Disability

  • Benefit - 60% of salary; paid monthly

  • Pre-existing condition clause - treated 3 months for a particular illness/injury prior to effective date of coverage, then that particular illness/injury will not be covered the first 12 months of coverage

  • Choice of Options:

    • 90-day Elimination Period; payable to age 65*

    • 180-day Elimination Period; payable to age 65*

  • Guarantee of coverage, if enrolled within 30 days of employment, otherwise, only during Open Enrollment and medical underwriting is required.  As a result, coverage can be denied.

*The long term disability benefits are payable for the period during which you continue to meet the definition of disability up to age 65, but not less than 5 years.  If your disability occurs at or after age 61, benefits would be paid for a reduced period of time.  See the chart below.

Age 61 48 months of benefit
Age 62 42 months of benefit
Age 63 36 months of benefit
Age 64 30 months of benefit
Age 65 24 months of benefit
Age 66 21 months of benefit
Age 67 18 months of benefit
Age 68 15 months of benefit
Age 69 and over 12 months of benefit

This is voluntary benefit and premiums are paid by the employee.  The premiums are calculated based on the employee's salary and date of birth.  They are deducted every paycheck, twice a month - 15th and the last day.
