| The District offers two (2) dental plans through
Delta Dental and they are DPO/Indemnity and DHMO. All
regular employees working at least 20 or more
hours per week are eligible to purchase this voluntary benefit. |
DPO/Indemnity

$50 deductible per covered
individual; $150 per family per Plan
year (April 1 through March 31)

The DPO/Indemnity has a large list of
participating dentists. However,
you are not restricted to one of the
participating dentist. Keep in
mind...if you go to a non-participating
dentists, you may have to pay the
difference between the charged amount
verses the approved amount.

twice a month - 15th and the last day. This is a voluntary
benefit and the premiums are paid by the employee, if coverage

DHMO
- No Claim forms to complete
- No annual deductibles and maximums
- Co-payments for all other services according to a schedule
- Two (2) cleanings per plan year
- X-rays once (1) per plan year
The
DHMO
plan
is
a
restrictive
plan.
You
must
select
one
(1)
dentist
from
the
provider
list
and
use
that
dentist.
If
you
do
not
see
the
dentist
you
originally
requested,
then
no
benefits
will
be
paid
for
any
services
you
have
rendered.
If
you
wish
change
dentists,
you
must
first
call
Delta
Care
at
800-422-4234
to
request
the
change.
